Accessing the HealthForms Website
- Users can reach the HealthForms website via a direct link or by manually searching.
- If the link does not work, follow the provided steps to search manually.
Instructions for Mobile Browsers
- Open the browser app on your device (e.g., Safari, Chrome).
- Tap the address bar and enter or paste the URL: healthforms.datavant.com/
- If redirected to search results, locate and select the correct link.
- If issues arise, close the browser and retry.
Instructions for Desktop Browsers
- Open the browser from your start menu or desktop.
- Click the address bar and enter or paste the same URL.
- Similar to mobile, if search results appear, find the correct link.
- Retry if necessary.
Completing Your Request
- Upon reaching the HealthForms homepage, click the "Get Started" button.
To complete the request, upload a driver’s license and any relevant documents in PDF format, ensuring multi-page documents are combined into a single PDF.

How to Search HealthForms
Step 1: Open Your Browser
The browser is how you will search the internet.
Find the browser app on your phone or tablet Home Screen.
Please tap on the browser you have on your phone to open it.
Your browser should look like one of the pictures below:
Step 2: Search For The Website
Tap on the address bar (usually at the top or bottom) and type in what you're looking for.
If you are successful,
You should see the HealthForms homepage and it looks like this:

Step 1: Open Your Browser

HOW TO COPY AND PASTE?
TO COPY:
Double click the link under “TYPE or COPY AND PASTE” on the previous page. Then, Right click your mouse on the highlighted text
And select COPY.
Make sure all of the text you want is highlighted by moving the highlight cursor as shown below:
TO PASTE:
Go back to your browser, Click your mouse in the Search bar and then Right Click your mouse and select PASTE as shown below:
(If it doesn’t take you to the website but instead you see a list of search results,
Look for the website you want and tap on the link.)

Step 3: You made it to the Website, Now what?
Click the blue “Get Started” button to complete your request and follow the instructions on screen.
Please Note:
In order to complete your request, You will need to upload
a copy of your drivers license and any documents you have
in pdf format.
Documents with multiple pages must be submitted as one
pdf document, if you try to upload one page at a time it will
only show the last page you uploaded.
