We know the process of getting FMLA, Short-Term Disability, or other medical forms can be confusing, so here are 3 important things you need to do.
Step 1: Notify your employer
Provide notice:
Inform your employer as soon as you know you need leave.
- If you know you need leave in advance (e.g., for a planned surgery), give your employer at least 30 days' notice.
- If the leave is unexpected, inform your employer as soon as possible - ideally on the same day or the next business day.
The goal is simply to provide notice as early as you can.
Note: If requesting for FMLA, you don't need to mention FMLA by name, but you must provide enough information for your employer to know the leave may quality under FMLA.
Step 2: Allow employer to respond
Wait for employer's response: Your employer must notify you of your eligibility within five business days of your notice.
Receive forms: Your employer will provide you with the necessary forms, such as the medical certification.

Step 3: Complete and submit the forms
Go to the HealthForms website, upload, and submit your forms. The link will be on the flyer that will be provided to you by your Healthcare Provider.
Allow Datavant 5-7 business days to gather the needed information from your provider, process your forms, and send them to your recipient.
