Q. What is HealthForms, and how does it work?
A. HealthForms is a Datavant application that helps you complete requests for FMLA, Disability, and other medical forms online. We work directly with your healthcare provider to fill out and process these forms on your behalf.
Q. Do I need to create an account to submit a form request?
A. No, you do not need to create an account to request form completion through HealthForms.
Q. What is HIPAA?
A. HIPAA is the Health Insurance Portability and Accountability Act of 1996, a U.S. law that protects the privacy and security of an individual's health information, known as protected health information (PHI). It establishes national standards for protecting electronic health transactions and gives patients' rights over their health data, including the ability to access, request corrections to, and control the disclosure of their information. HIPAA also includes provisions for health insurance reform, such as protecting coverage for people who change jobs.
Q. Does the HealthForms team process requests for medical records?
A. The Healthforms team does not process requests for medical records. If you need medical records, you can:
- Request them from your healthcare provider.
- Obtain them from your patient portal (if your healthcare provider has one)
Q. If I do have an outstanding medical record request, how can I check the status of that request?

Q. Can I use HealthForms if I am a Caregiver?
A. Yes! However, you must have the legal authority to act on behalf of the person for whom you are submitting the form. Caregiver documentation is required to confirm your authority to act on the patient's behalf (some options listed below)

Q. How can I check the status of my HealthForm request?
A. You can visit the HealthForms Status Page at any time using the confirmation number that was emailed to you shortly after submitting your forms on the HealthForms website.
Q. Who are the recipients of HealthForms?
A. The recipient would be the person or entity that the patient wants their completed documents sent to. This is commonly their employer, or a 3rd party that manages their FMLA and/or Disability paperwork.
Q. How are HealthForms Processed?
A. Once we receive your HealthForms request:
- Datavant fills out the forms.
- Your healthcare provider answers any questions we have and signs the completed form.
- Datavant delivers your forms to the recipients requested.
Q. What is the estimated turnaround time for your forms to be completed?
A. We estimate 5-7 business days because we may need to work with your provider to get the necessary information to accurately complete your documents. If all the information we need is present, then a simple FMLA form can be completed within a shorter timeframe.
Q. Who completes and signs the forms?
A. Datavant completes the clinical information on the forms and queues the completed form for the Provider's signature. The Provider will sign the completed form.
Q. How can I request an update, get an extension, or follow up on a request?

Q. If I need to email support, what information should I provide?
